Because they dont know about the Plum Bacon Pizza... Taste is just a personal preference but people enjoy to ignite the most deeply instincts by trivial things.
As my long ago roommate from Long Island stated as she opened the pizza box to find ham & pineapple "That's not a pizza! You don't put fruit on a pizza." We're still good friends to this day.
Pizza with nutella and strawberries is definitely something you van have in Italy.
My complaint with pizza with ham and pineapple is that it doesn't combo that well. I have no problem with the pineapple, but the combination doesn't enhance the pizza flavor, there are better combos out there.
Also notice that a good quality margherita pizza had an incredible taste thanks to the tomato sauce.
Most tomato sauce I tried in north america are terrible.
I agree with you about ham and pineapple. The problem is the sliced ham, which just doesn't pair well with tomato sauce, not the pineapple. Maybe taro would be a better topping than ham to make a pizza unambiguously Hawaiian.
I am not Italian, but I enjoy pizza. You might be interested to know that (ham and pineapple) Hawaiian pizza was the invention of a Greek-born Canadian.

I'm pretty sure a pizza with pineapple is workable, after all there is one asian rice I have been served which included pineapple and was amazing
Pepperoni, pineapple, jalapenos, tomato sauce, full-moisture buffalo mozzarella. Spicy, salty, sweet, acid, flavourful, absolutely great. Far better than the sliced ham you often get on pizzas, which is mostly flavourless and not even particularly salty.
And jalapeño, bell pepper, olive, lemon (in pesto), fig, and corn, among others. People who reflexively reject pineapple on pizza aren't true pizza lovers.
I had a pizza in Sweden once with banana, seafood and curry powder and honestly it was better than most of the authentic Italian pizzas I have tasted.
